Medical billing, revenue cycle management and coding: what do you cover?
Northgate covers every step of revenue cycle management after the visit. We bill from the codes your providers assign and catch errors before a payer sees them. Coding from clinical notes and payer enrollment are separate, and priced before you say yes.
Covered by the rate
- Eligibility and benefits checks
- Charge entry, scrubbing and claim submission
- Denials, appeals and payer calls
- Payment posting, A/R follow-up and patient statements
- Code edits caught before submission
Separate, priced first
- Coding from clinical notes: a coding review add-on, quoted before you say yes
- Payer enrollment and CAQH: our insurance credentialing services, one flat fee per provider
Outsourced medical billing services: when does it make sense?
When billing is costing you more than the fee would, in staff hours, unworked denials or cash that arrives weeks late. When your in-house biller is on top of it, keep them.
It usually pays when
- Billing is eating staff hours you do not have
- Denials sit unworked because nobody has time
- Cash arrives weeks after the visit
- Your biller is leaving, or is the only person who knows the process
It may not when
- You have a strong in-house biller with time to work denials
- Your denial rate and days in A/R are already low
Send a 90-day aging report and you get a written audit back in 3 business days: denial rate, days in A/R, and what you are paying per dollar collected. Is outsourcing medical billing a good idea? Run the free audit
How much do our medical billing services cost?
A percentage of what we collect, so we are paid when you are. 7% for the smallest practices, stepping down to 3% as collections grow, with a $450 per provider per month minimum. Denials, appeals, statements and reporting are in the rate. No setup fee, no onboarding fee, no statement fee. DME and HME, EMS and ambulance, and home health carry a complexity factor, shown on the Pricing page and in your agreement before you sign. See full medical billing pricing.
Do you work in my software and my specialty?
Yes. We bill inside the software you already use, so your data never moves and nothing has to be migrated. Nine specialties have their own billing playbook; most other outpatient specialties run on the same model. Solo provider or growing group? See medical billing services for small practices.

Questions practices ask before handing over billing
What does a medical billing service do?
A medical billing service verifies coverage, submits your claims to insurance payers, works denials and unpaid claims, posts payments and sends patient statements. Northgate does this inside your existing software, so you keep your data and can see every claim.
Is outsourcing medical billing a good idea?
It often is when billing is costing staff time, denials are climbing or cash is slow, and often is not when you already have a strong in-house biller. Northgate's free billing audit uses your own numbers to show which applies.
How much does a medical billing service cost?
Rates vary, and the lowest advertised ones often leave out denials, statements or setup fees. Northgate charges 7% of collections stepping down to 3%, with denials, appeals and statements included, and a $450 per provider per month minimum.
Do I have to change my software?
No. Northgate works inside the system you already use, so your data never moves.
How long does switching to Northgate take?
About 30 days from signed agreement to your first review, including a parallel month alongside whoever bills for you today.
Do you use AI, or do people work my claims?
Both, in different places. Claim scrubbing and eligibility checks run through software inside your system. Denials, appeals, payer calls and your weekly report are handled by your account lead and their team.
What does my staff still do?
Check patients in, collect copays and document as usual. Northgate takes over everything after the visit. Nobody at your practice loses access to anything.
Is Northgate HIPAA compliant?
Yes. Northgate signs a BAA before touching a record, works inside your system rather than exporting your data, and every team member completes HIPAA training. Details are on our HIPAA compliance page.
