How the brackets work
Each slice of your collections is billed at its own rate, the way tax brackets work. Growing past a line never raises the rate on the money below it: no price cliffs, nothing to renegotiate.
| Monthly collections | Rate on that slice |
|---|---|
| First $25,000 | 7.0% |
| $25,000 to $50,000 | 5.0% |
| $50,000 to $100,000 | 3.6% |
| $100,000 to $250,000 | 3.5% |
| $250,000 to $500,000 | 3.25% |
| Above $500,000 | 3.0% |
| Practice | Monthly collections | Monthly fee | Blended rate |
|---|---|---|---|
| Solo therapist | $20,000 | $1,400 | 7.0% |
| Two-provider chiropractic clinic | $45,000 | $2,750 | 6.1% |
| Three-provider PT practice | $90,000 | $4,440 | 4.9% |
| Eight-provider group | $200,000 | $8,300 | 4.2% |
| Multi-site group | $400,000 | $14,925 | 3.7% |
Rates are set on your trailing three-month average of posted collections, recalculated monthly. Grow and your blended rate falls on its own. Dip for a quarter and the minimum protects us while the brackets protect you.
The two things that can change the number
Minimum fee
Applies when 7% of a provider's collections would be under $450, roughly $6,500 a month. Below that, the work does not shrink with the revenue.
Specialty factor
DME and HME, EMS and ambulance, and home health: many low-value claims per dollar collected, plus documentation and prior authorization work. Shown in the calculator and on your agreement before you sign.
No setup fee, no onboarding fee, no statement fee, no per-claim fee, no charge for appeals, no charge for reporting, no charge for calls.
Credentialing services pricing
One flat fee per provider covers your first eight payers, CAQH setup and attestation included, no setup fee. What the fee covers first, then the fee.
What is included
Application preparation and submission, CAQH setup and attestation, follow-up with the payer until the contract is signed or the panel is confirmed closed, a status sheet updated weekly, and a copy of every submission.
What is not included
Payer-side fees and state license fees. You pay those directly.
If a panel is closed
We tell you in writing and you are not charged for that payer.
| Item | Price |
|---|---|
| Initial credentialing and enrollment, per provider, up to 8 payers | $1,200 |
| Each additional payer beyond 8, per provider | $120 |
| Re-credentialing, per provider | Price pending |
One new provider joining five payers: $1,200, because the fee covers up to eight. One provider joining ten payers: $1,200 + 2 × $120 = $1,440. Paid 50% at submission and 50% at approval. If a panel turns out to be closed, that payer is credited back.

How our pricing compares with other models
Published industry guides put percentage pricing at 4% to 10% of net collections, per-claim pricing at $3 to $12, and flat fees at $500 to $2,500 a month. The headline number matters less than what it leaves out.
| Model | Typical range | Where it goes wrong | Northgate |
|---|---|---|---|
| Percentage of collections | 4% to 10% | Low headline rates that exclude denials, statements or appeals, with setup or onboarding fees on top | 7% to 3% by bracket, everything included, no setup or onboarding fee |
| Per claim | $3 to $12 per claim | Pays the biller to submit, not to collect. Denial work is often extra | Not offered. We are paid when you are |
| Flat monthly fee | $500 to $2,500+ | Fine for stable volume, penalizes growth and seasonality | Only as the $450 per provider minimum |
| In-house biller | Salary and benefits | Turnover, coverage gaps, software and clearinghouse costs on top | A named account lead and a team behind them. Use the guide below to compare the fully loaded cost |
They usually apply above $500,000 a month, exclude denial work and statements, or add setup, onboarding or software fees. Send us the quote and we will compare it line by line, in writing, at no cost.
How and when you pay
Is percentage-of-collections pricing allowed?
Percentage-of-collections is the most common pricing model in medical billing. Some states regulate fee arrangements for certain payer types, and government programs have their own rules on how billing vendors may be paid. Where a percentage arrangement is not permitted for part of your payer mix, we bill that portion on a flat per-provider fee instead. This is general information, not legal advice. If your counsel wants to review the agreement, we will walk through it with them.
Pricing questions practices ask before they sign
What percentage do medical billing companies charge?
Published industry guides put it at 4% to 10% of net collections. Solo and low-volume practices tend to sit at the top of the range, large groups at the bottom. Northgate charges 7% on the first $25,000 a month, stepping down to 3% above $500,000, and the rate includes denials, appeals, statements and reporting.
How much does medical billing cost per month for a small practice?
A solo practice collecting $20,000 a month pays Northgate $1,400. A three-provider practice collecting $90,000 pays $4,440, a blended 4.9%. The minimum is $450 per provider per month. More examples by practice size are on our page about medical billing services for small practices.
Are there any setup fees or hidden fees?
No. There is no setup fee and no onboarding fee. There is also no statement fee, no per-claim fee and no charge for appeals, reporting or calls. If we ever offer an add-on, you see the price first and say yes or no.
What is the $450 minimum and when does it apply?
It is the least we charge per provider per month. It applies when 7% of that provider's collections would be under $450, which is roughly $6,500 a month in collections. Above that, the brackets apply and the minimum never does.
Do you charge per claim or a flat fee?
No. Per-claim pricing pays a biller to submit, not to collect. A flat fee penalizes growth. The percentage model means we earn more only when you do, and the $450 minimum is the only fixed element.
Is the rate based on gross or net collections?
Net collections: money actually posted to your accounts from payers and patients in the month. Not charges, not billed amounts, not contractual write-offs.
Why is credentialing priced separately?
Credentialing is a project with a defined scope: one provider, a set of payers, a contract with each. A flat fee per provider, covering up to eight payers, means you know the total before we start, and it does not depend on how much you later collect.
Can I switch to the 12-month rate later?
Yes. Any month-to-month client can move to a 12-month agreement and the lower rate applies from the next invoice.
